In Massachusetts, licensing is split across more than one board, and MassHealth has its own enrollment application with specific rules about service locations and paperwork dates. Here is how those pieces fit together for therapists.
Licensing: two boards, depending on your license
The Board of Registration of Allied Mental Health and Human Services Professions covers fields including mental health counseling, marriage and family therapy, rehabilitation counseling, educational psychology and behavior analysis. Social workers are licensed by a separate body, the Board of Registration of Social Workers, which lists LICSW, LCSW, LSW and LSWA licenses. Each board has its own application page on Mass.gov, so start with the board that matches your license.
MassHealth enrollment
MassHealth enrollment starts with an application package requested through the "Apply to become a MassHealth Provider" page. Details worth knowing before you start:
- Individuals who practice within a group, or who bill directly while practicing independently, enroll as fee-for-service providers.
- Sign the forms only when you are ready to submit. MassHealth does not accept signed forms after 90 days.
- You track the application with its Application Tracking Number (ATN) or your NPI. MassHealth aims to process applications within 30 days, and if something is missing it contacts the person listed on the application.
- Groups must enroll each service location where members are seen. Administrative offices cannot be enrolled.
- If a Massachusetts license is still pending, the application can be pended for up to six months. After that you reapply.
- Tell MassHealth at least 14 days before a change in your information.
Licensed independent behavioral health clinicians
MassHealth refers to independently practicing behavioral health clinicians as licensed independent behavioral health clinicians (LIBHCs). A 2025 bulletin says that effective October 1, 2025, they no longer need to be Medicare providers to enroll with MassHealth, though other requirements in 130 CMR 462.404 still apply. If you treat members who also have Medicare or other insurance, you must be enrolled with that insurer too and follow MassHealth's third-party billing rules.
Check the regulation and current bulletin to see whether your license type is included before you apply.
After you are enrolled
MassHealth Provider Self-Service lets you check application status and enrollment dates and request changes. The Provider Online Service Center (POSC) is where fee-for-service providers bill, submit prior authorizations and referrals, and download remittance advices. After you are approved, MassHealth mails a welcome letter to your service location address and asks you to complete a Data Collection Form to set up the primary portal user.
Where Massachusetts applications tend to stall
- Forms signed too early and then expired at the 90-day mark.
- A group that enrolls its main office but not every location where members are seen.
- Listing an administrative address as a service location.
- A letter that goes to a service address nobody checks, so a welcome letter or request for information sits unread.
MassHealth's 30-day figure is a processing aim, not a guarantee. Commercial plans in Massachusetts run their own separate credentialing, which we scope with you.
